Basic rules

By sending this form, you express your consent to provide your personal data under the following conditions:

By clicking “send registration for approval”, I confirm that the personal data I have provided are accurate and true, and I agree to have them processed by the company PROMEDICA PRAHA GROUP, a.s., with registered address in Prague 4, at Novodvorská 136, Business ID: 250 99 019 (hereinafter referred to as the “administrator”), and used for the purposes of the company and for marketing purposes within the scope of activities of the administrator for an indefinite time period (i.e. until I withdraw my consent).
I understand that provision of personal data is voluntary, that I have the right to access my personal data in accordance with Act no. 101/2000 Coll., on Personal Data Protection, I also have the right to appeal to the Office for Personal Data Protection if my rights are breached, and I have the right to withdraw my consent to use my personal data at anytime.

Additional conditions

Registration is open only to the users – representatives of a company from the groups of existing or potential clients of PROMEDICA PRAHA GROUP, a.s.
